THE TRANSFORMING CARE PARTNERSHIP WITH ASPIRE IN THE COMMUNITY: LIVERPOOL

The Transforming Care programme is NHS England’s plan to ensure that people with a learning disability or Autism do not have to remain in a hospital setting when their behaviours cause concern.

Our centres are based in Yorkshire but we currently care for residents that have come from the all over the UK. Liverpool is roughly an hour and a half drive to our residences.

NHS England has committed to reducing hospital beds, in favour of providing specialist community services, where people can be supported by trained and skilled staff, with support from local Intensive Support Teams.

We know that lots of people often remain in hospital for long periods of time, despite having no mental health diagnosis. Autism and Learning Disability are not mental health conditions but often people who have unmet needs, display behaviours of concern due to a lack of development in social and communication skills.

Often people, who display behaviours of concern, require a specialist environment which helps them to be safe whilst being supported by staff who have a good understanding of Autism and why behaviours may be presented.

We are pleased to be able to offer specialist provision to enable people to be discharged from hospital or to prevent hospital admission, in partnership with or local Clinical Commissioning Group. We work with professionals from a variety of disciplines to ensure that we can meet the holistic needs of people who require intense support.

It is our hope that in future, the needs of all people with a learning disability and Autism will be met in a community setting, in order that they may continue to benefit from accessing their local community and maintain relationships with friends and family.
